Don't forget that the golden age of the traditional Telephone company was in the 1940s when it first became a critical domestic part of the "War Effort". For the next 30 years, the style of management and organization was based on military principles adopted during those war years. It was considered efficient and supportive to follow the that military model with its chains of commands, multilayered management, micro-documentation of every task.
Keeping that same mindset, now add technology with the ability to monitor every moment and movement of the day and you end up with Managers of the Microscope, so afraid of missing some nuance that they instead eyeball every molecule of their universe looking for something you did wrong.
None ever look up and out and see the big picture. Too many forests of people looking at leaves.
None see the sky above or beyond.
