I have a certified letter from AT&T waiting at the post office for me. What could it be?

It was a letter stating that they were going to take my wife off my insurance because she wasn't verified this year... I have had to verify her every year for the last 3 years! And yes I had sent the paper work in.

I don't have kids, back taxes, alimony and I pay my student loans faithfully. I haven't been sued by anyone that I know of so I don't know who would try to garnish my wages.
