Does anyone have the address to send the elections form to for COBRA acceptance. I accidentally tossed the envelope it came in and don't know where to send the form.

@ad it’s not February for everyone. Most people’s end December 31st
No need to opt in unless you wanted FSA or one of the other minor benefits. For medical and dental, Nike auto enrolled you.
It looks like they pre-paid it already so you don’t have to do anything until February. You login online and there isn’t anything to opt into at all.
