If you worked at U.S. Bank anytime since 2019 and had health insurance through them, you may have been charged extra fees — sometimes $50–$100 per month. These surcharges were added for things like smoking/vaping, keeping a spouse on your plan, or missing certain wellness checkups.
Attorneys are now investigating whether those charges were legal. Employees could be owed compensation if the surcharges violated federal or state laws.

I’m fine with smoking and vaping surcharges. They are unhealthy people.
Why don’t they charge a surcharge for over eaters
Well I worked at us bank from 2017 - 2024 and was charged the fee for smoking as I should be my spouse who was in my insurance at the time was also charged for the fee of smoking only thing is he never smoked I tried to get them to listen to reason but no one ever did anything about it for the entire year I know peeps think I hit the button for him as well I swear I didn’t and his primary doctor even sent in documentation a testing to him never smoking so yes I’d love to see that back I had other issues but I won’t go into detail here
Tobacco surcharges for smokers are normal under most employer funded insurance plans. There are cash incentives for joining smoking cessation programs and utilizing wellness checkups. I am not sure about the fee for spouses being eligible to use their workplace plans.
Get a grip. Quit smoking and also every plan now prevents a spouse from being on a plan if they have a job that offers insurance also.
Nobody cares about smokers
