Is the bonus percentage in your offer fixed annually until you change job with different bonus percentage?

It used to be, but now that bonus targets have been discontinued nobody knows for sure but I would bet no.
Bonuses are not guaranteed. There have been years where they were not paid out at all (see WFHE circa 2008). If paid out, it can vary year to year by several percentage points.
This year is gonna be special.

I found that over the years, it’s whatever your leadership team decides.
