Questions about what I was doing to prepare for RTO, though she's fully aware I have a medical exemption. Questions moved on to asking if I have been thinking of leaving WF and why?
I have been involved with this company for 11 years and have never experienced this before.

Contact an attorney and ask what your options are in terms of an age discrimination lawsuit. Or if you may be able to negotiate a better severance.
If asked, everyone should say if im forced to return 5 full days, id leave. otherwise a cut in pay is the only other issue that would force me to leave. Then this will be a clear message to the OC we aren’t willing to do full RTO. Those same three weirdos that keep claiming RTO is so awesome bc they don’t like their wife and need an office won’t stand a chance against the rest of us. lol

Simple solution to avoiding RTO. Change your primary address to a strip mall mailbox company at least 45 miles away from the nearest WF location.
They are prepping for full 5 day RTO and the survey is to determine what attrition, problems, etc may occur. While I love wfh I’m smart enough to know it’s not going to last forever.
No mandatory surveys, for years, from what I’ve seen. I do recall one team issuing a survey to us. They neglected to add the ‘not applicable’ option for us. Don’t remember the questions, but I do recall, they were kind of a set up. For example, how often do you exceed your PTO allowance?
There was no n/a option to choose from. It’s like the leadership just assumed we cheated on PTO allowance.

You’re being set up. Did your manager ask if there was anything he/she could do to help support you in RTO given your documented exemption? If you’re planning on staying, the only answer is “I have no immediate plans to leave.”
yep. this is the latest self-delusional action the OC has decided to take to pretend they actually care about keeping people happy in the workplace and attrition. They are interviewing people who are still here about why they stay, ostensibly with the rationale being that they can then understand what people value about working at WF and keep doing that, while they can then ferret out any other themes about why people might be thinking about leaving. Like an exit interview, but done before people exit.
Your manager is crafting a story to have you gone because she does not want you for some reason. You may be smarter, or more attractive, or better thought of in the org. In the end, none of this matters as this manager will win. My words to this manager would be - careful you will reap what you sew. God bless and best wishes to you.
