What do you have to do to get promoted? I've been working 60+ hour weeks, finishing every extra task I was given, I was always there to help out when an extra pair of hands was needed and I've always had a good relationship with my manager. What I got in return is more work and assurances that they'll keep me in mind for the next promotion - which I once again didn't get. I feel like a complete fool right now.

If you aren't a favorite you won't get promoted. I know leaders, managers, directors and seniors that wouldn't be able to pick their nose without bleeding. They got their by being a "yes man".
It’s a low-mediocre people shop. Someone like you who can’t find any better place to work are working here. Gain some self respect and move on.
You’re too valuable as a front line associate. Management can’t do your job so they’re going to hold you hostage. Transfer or leave. Then it will take 4-5 people to replace you!
Or, they’re keeping you in your current position because you’re too valuable to lose
You gave yourself the wrong image. You have shown people that you are task driven.
It’s very hard to move up in Humana, lots of competition
Unfortunately, this seems to be the way of the world right now and the only way for you to get some well deserved sanity is to move on to another organization. Everyday, higher ups are taking their golden parachutes elsewhere and leaving all the work for the rest. Only you can make yourself happy and now might be that opportunity. Have faith in you and move forward. Plus, maybe you'll get an actual raise. Unlike the current insanity going on. Good luck to you.
