I would complete the self evaluation and be honest. That's where you have the chance to call things out. People think it's a waste of time but in every single role there are things we do or things that are done to us that others have no clue about.
Just make sure when you complete the self evaluation do it correctly follow the instructions.

If I received a review for an underling from her underling, I would do this:

  1. Send the review to my underling saying "fix this however you want, but make it stop."
  2. Back my underling up on however she decided to deal with you.

See, I hired my underling. And my underling hired you. That means I know her, but I don't know you. And if there's a conflict that isn't something you could sue us for, I'm gonna expect her to terminate you if your relationship is so out of control that she's bothering me with it.

If this is the second time, however, I'm gonna take a closer look.

The moral of the story is: the late mouse gets the cheese. Don't be the first one to complain. Ever. You want an ocean of drama that's made your boss more headache than she's worth for her boss before you try a powerplay like this, and there are better ways like an in-person discussion with your boss's boss over coffee.

The CP&D tool will be open for employees on 9/27. You can add your comments and I don't believe your manager can change it.

You can always do that, especially if you have existing 1x1s with that leadership. Send to mgr and cc their mgr.

My question for you is … why not find a new job where you respect the manager and can have a trust-based working relationship?

Great talent will always have choices where they work and who they work for.

every manager 2 or 3 up always says "open door policy" but whenever I've emailed them i always get in trouble for not following "chain of command". they always want you to go through your direct manager.

Probably, but it won't do you any good. It'll just single you out as a trouble maker.

Remember that HR and your 1-up (and higher) are all on the side of management. They see the workplace as an "Us versus Them" environment, and you are automatically seen as the opposition in their eyes.

Best practice in this situation would be to quietly do everything in your power to undermine your immediate manager, and recruit others to that effort as well.
