It's in the management severance plan. Please go to hronestop, search for severance, then search the severance doc for MTP. You will also want to check the retiree benefits doc to see the date your original hiring company stopped offering subsidised retiree health care. Pacific Bell was 8.2.1997.
Also, search this site for MTP and health care subsidy and AT&T. Lots of good info posted by people who went through the process.
