Actually admissions and pdcs can be and are really full of social work needs. More so than a regular moc call. Maybe we will be able to refer to a nurse if there are nurse needs and vice versa.
SNP social workers aren’t really doing social work, sounds as if they’re still doing the nurses role
TOC stands for transition of care
PDC stand for post discharge calls. So basically calling members before they go into the hospital and again when they are discharged from the hospital and doing surveys

What are TOCs, and PDCs
It sounds like SW will be doing TOCs, possible PDCs, and possible ICT support for social work type needs. There will also be a group of us going to Auto Dialer to work on re-engagement of Members. We havent been told more than that. We dont know a start date or exactly what we will be doing yet. I know we wont have rosters which I'm happy about because the roster sizes have stayed around 850-900 and it was way too much to do for one person. They really need to hire more people, and fast, because SNP is getting a 1/3 increase of members soon. They also need to work on retention this time around.
What will the SNP social workers be, if not the care managers? Would they be going back to their earlier more appropriate role?
