Live healthy describes a level of intervention within HCCP. There used to be Stay Healthy, At Risk, Health and Functionally challenged and High severity Telephonic. Stay Healthy went away last year, At Risk was re-branded as Live healthy. That will now go away. HCCP will remain but all members will have an higher clinical need than those who were in Live Healthy.
HCCP is not ending but it is changing. Only members who can benefit from an RN intervention will be managed, most others will be monitored. They will move back to managed if they have a qualifying event.
I hope for those of us remaining that this works. I don't want to drag my happy butt into an hospital everyday.

I think Live Healthy was just PR bull to not say Stay Healthy was shutting down.

Live Healthy was a combination of Stay Healthy and HCCP. HCCP and Live Healthy are synonymous.
I work live healthy with HCCP mbrs. So maybe live healthy will no longer exist but with the new model it will just be HCCP. Since we are moving to a call schedule. Just my guess
The only PHCs are live healthy. HCCP was folded into Stay Healthy this year and it is all one department now.
HCMSS here. Can someone please explain to me the difference between HCCP and Live Healthy? We were previously considered HCCP. We then absorbed Stay Healthy and were told we're now 'Live Healthy." There has been some discussion of HCCP and Live Healthy still being separate on previous threads. I know there is much speculation that the PHC role will be eliminated, but if Live Healthy is gone, wouldn't that mean all of us SS, RNs and CHE team previously designated as HCCP would also be gone??
