LTIH is folding...true? Will they keep RN FCMs for another program?

Maybe hcmss can refer to LTIH in the future, they’ve been asking to be able to for awhile, they really can identify the need for psychosocial support and face to face, if humana is doing more in home, it would make sense, since hcmss are also care planning, if they could refer to LTIH, too

The email does say new members or transitions will go to LTIH 2. So, when I see bigger lists of LTIH, then I’ll believe that it will continue. Right now, barely nothing. You know all of those chronic disease members are affected by the flu that’s blazing through the country with their chronic disease flaring. I’m guessing they are restructuring or there are not going to be NEW LTIH members just existing. Although, w/o the staff to refer all of those members for the programs.....so, Restructuring or phasing out. I can’t tell.
LTIH 4 is going to be no more. The rest of LTIH is staying. The program is NOT going away.
The email said move to 2 visits with EXISTING members....so if no new members....sounds like phasing out.
I have 35 members currently! I am wore the f--- out! My ME is nearing 42%. I have a mix of LTIH 4, LTIH 2, transitions and tele's My area certainly has enough and is not lacking LTIH members. I am still not confident. :(
