So now LTIh will be moving to monitored. Guess we know where those additional cuts in January are going to be now.

No, previous poster, it does not. LTIH goes from 4 visits a month, to LTIH 2 which is 2 visits a month, to telephonic, all managed by the same care manager. Telephonic does not go back to having visits if needed, or so we were told. Once telephonic members go monitored, they are off our rosters. Where they go we have not heard. We are to work toward graduating our members out of the program because that means they are improving. It also means we are working ourselves out of a job with no members being referred.

All it means is that the fees members who no longer need in home visits or 2x month calls will be monitored in LTIH rather than discharged. It's for continuity of care. So when/if the needs to move back to visits arises, they are still part of LTIH.

Yes LTIH HCM, I think you are right. Feels like the end of the road to me. You can't keep HCMs with no work. I believe our day is coming very soon. Uggggh!

Best of luck to you as well. I loved what I was doing and felt like I was really able to help people in their homes with community resources as well as health education and coordination. Right now I am pasting a smile on my face while continually looking for other nursing jobs. It's just feels way to shaky right now to trust in the job being there much longer.

What would LTIH monitored mean? Would monitored mean when they move to phone calls only or is that LTIH 2? How many people remain in LTIH after the last round? I'm in HCCP and am trying to understand how it works on the LTIH side.

HCCP MON Nurse Triage Team will address alerts and moved back to managed prn. Yes, lots going on as I mentioned about cuts, but they also said they want to retain nurses. Maybe that was true, just until they get down to the bare bones of operation for 2.0.
