For management, is your remaining PTO for the year paid out or is it lost?

No. So you wnat to make sure you use them all up first.
In management and you are supposed to be paid out unused earned PTO. In my case I used my full year and was terminate early Oct and got my last paycheck and they didn't deduct for using unearned PTO.
There is a table that shows the number of days you earn every month. It is 3 in Jan, 2 in Feb, 3 in March. The pattern repeats for every month, earning you 7 days every 3 months. You get paid out the number of days you earned minus the number of days you took.

Do you have to pay them back for any vacation time you took that hadn’t been accrued yet?
What you’ve accrued is paid out if unused.
Which is why 4th quarter separations are so popular--they cost less. Bonus perk for them is they get to sc--w you over in time for the holidays. (New Jersey, we're looking at you.)
For bargained if you are rule of 75 you get your full year PTO paid out.
