would i get a black mark on my profile if i was honest in the survey and revealed how my manager is completely and utterly useless? and also a sex1st and rac1st?

Speaking as someone who served on an advisory board for the Pulse surveys, I can tell you that they aren't as anonymous as they purport to be. Your name will not be attached to your comments, but your pay band, title and group will be. If you're in a large group with 30 other people who have your band and title, that's not enough to identify you. But, if you are one of a couple people in your group at that level, it won't be hard to identify you. And, even though retaliation is 100% illegal, in the words of my Business Law professor..."What's legal is irrelevant. All that's relevant is what you can prove". So be careful.
But - I do agree with the other commenters about sexism and racism. If you have examples of those kinds of actions, take them straight to HR and bypass the Pulse survey.
They will know which group it came from and for potential illegal activity will investigate. That said, be honest - especially in the verbatims. I always put in comments re: if my immediate leadership is great (He is!) or not as opposed to the VP and above leadership and constructive specific items that get in our way to do business. Dont bi--h and moan.

Back in the days they had STI awarded to you in your checking account, a coworker made an earlier complaint against our Manager. That resulted in the Manager losing their STI award that year. And it was mentioned by the Manager to the coworker during the "Here is what your STI is" discussion. So they don't forget, and as long as you have that Manager, that's always going to hang in the balance. Better just to apply for another position.
I would not be honest. You will pay the price. I'd also not speak to your manger about it. They're not going to change and there will be problems for you.
If your manager is sexist or racist you should follow the guidelines for adressing those. Talk to your manager about the incident(s)/behavior(s), if you're uncomfortable doing that, go to their immediate manager/director, if you're still not comfortable with that go directly to your HR rep. Be prepared to cite specific instances and examples, if you have it documented in any way bring that (remember though, that recording, photographing, or taking video of another employee at work without their consent is against the Credo). If others in your work group have experienced the same thing, and you feel comfortable talking to them about it, you may want to ask them to go with you to talk to whomever you decided you were comfortable talking to.
Be honest on the survey, if you have a $hltty manager, and they retaliate, take that to their manager or HR as indicated above.
You won't get a 'black mark' but your manager may not like you if they tie the feedback to you, and if your manager is $hltty, you may see that reflected.
