Has anyone else heard that Humana at Home is changing to Humana Care Management? Many assumptions can be made. None of them good.

Do you know when those changes will be announced?
They are making changes region by region combining HAH and regional case management departments and making many cuts in staff along the way. It’s very possible some have not heard about the changes yet and what it means for regional teams.
Below poster, yes your information is very helpful. Essentially the piece of the organization previously called “Humana At Home” is now called “Humana Care Management”. It’s significant that they dropped the “At Home” part of the name and implies there must be a reason it was changed to “Care Management”.
FYI, Humana Care Support is under the Humana Care Management org/umbrella. Care Management is the new name. For those saying the new name is Humana Care Support, you are confused. Humana Care Support was under Humana At Home and is now under Humana Care Management.

To the poster that said “we are in the transitional phase of it” that must be the phase of determining if the program is worth keeping. The way ECOM/Humana Cares is going Im pretty sure it will be on the chopping block as part of the 1 billion in cuts that are coming.
Old news. Where have you all been? Living under a rock? Humana at home merged with market case managers and is now Humana care support .

When you put all the pieces together (Kindred purchase, the push for in person care management to focus on SDOH, decreased member enrollment, falling stock, etc) it doesn’t leave much room for telephonic care management when trying to cut 1 billion dollars.
I am in HAH. The name changing is news to me as of this week!
This is not new news….the ECOM pilot was and us specifically for this purpose, and in those states, and the model has been up and running using sales force for close to a year….telephonic support, with teams that include nursing, pharmacy, social work, behavioral health…it is in house, not out of Centerwell….HAH is morphing to Humana Care Support….we were all told of the new name a few months ago.
What does that mean for WAH telephonic nurses?

Based on this article HAH has arrived at the end. The telephonic positions will now be handled in the primary care clinics that are already addressing SDOH. Guess that makes sense with the big CenterWell expansion that’s happening.
