Getting education on how to manage your health issues and when to report changes in signs and symptoms to your doctor with definitely prevent readmissions and members will live healthier lives.

Nurses will Never impact, nor keep members from going to a hospital and being admitted, by an ER physician. We live in a very litigious world, where a doctor nor hospital, are willing to take a risk not admitting a patient. That’s why frequent flyers are always admitted. Humana nurses nor the insurance company, will Ever, change this.

If you are not doing any education with these post discharge members and high priority members with chronic conditions you are not doing your job.

There is no nursing in any of these calls. Humana wants us to coerce our members to do medicine reconciliation to bump their star scores. So pathetic.

It’s the demographic they are dealing with in the telephonic model, which is soooo different than years ago. The bottom line is doing post discharge gap closing at a rapid pace with the goal to prevent readmission in a smaller and smaller time frame
