So humana plans to focus on post discharges from hospital? Transitions program has been nothing but a huge stressful nightmare for every RN since Humana dumped their program last year! Bungled mess run by a group of absolute buffoons but now every nurse except utilization and quality will be doing post discharges? Id--ts! Get out while you can still go!

Kindred can do the in home visits
UM does discharge planning, they don’t outreach to members only to hospitals and PCP
They are focusing on post dc initially. Then opening up additional kinds of calls over time and realignment. It won’t be just post dc forever
Well true Transitions post hospital takes many forms. I meant the in-home visits following a hospital stay were very valuable in my opinion.
Below poster, so if they are going into regions, and transitions is still up and proving it works, are they just scaling up transitions, and calling it something else? That would mean those folks are safe.
Transitions is still going and it is all telephonic. We recently had really good numbers with members and preventing readmission- probably why they are going to continue in post discharge area. T

Insane to me! Through customer service and caring for members was tossed aside with the end of the transitions program.
Interesting..the Transitions program used to so valuable…we truly made a difference to members..community support was valued…Sometimes I wonder if leadership really knew what we did in the homes? Did they really understand what we were doing in the difference we made? The education we focused on…The personal attention we gave to these members. The information to the various benefits that we provided to them.
