I work in a position you cannot do from home. I have been out on vacation and traveled out of state on an airplane. My supervisor has also been out on vacation and unavailable. I do not feel 100% but my symptoms right now are so mild I might just need water and rest from a long week. There is still the chance though so I am not sure what I should do.

Based on how the company disciplines for sick time, just go to work and tell management to go f themselves...
You should contact another manager if possible or you can submit to be off for the 14 days they are offering. Keep in mind they are requesting doctor information and when your appt was and what the doctor said. To submit or file for it you can visit hr one stop.
