My position was eliminated in October 2015. I had health insurance coverage for through April 2016. Should I have received a 1095-C form to show that I had coverage? I am asking because HR said to contact Kaiser and Kaiser said to contact Chevron.
Please advise.

Eventually you should get a 1095 C form. May take some persistence. In the meantime, keep documentation showing you paid for insurance and had coverage.
OP, During those 6 months after being laid off from Chevron, you were not covered under employee coverage anymore, so Chevron is not the place to go. You were covered under COBRA. Even though Chevron may have been paying the usual contribution on your behalf, the administrator of your medical insurance coverage was ADP Benefit Services. You should call them and request a copy of the Form 1095. If you still access to your online account at ADP, check the documents section to see if it's there for you to download or print. If not, you will have to call them.
If Chevron is self insured, then it would come from Chevron.
