Four or five feet is a good over excavating width for working on forms.
It is possible the construction crew dug through the line.
If the well was shut in at the time there may have not been a noticeable sustained release of pressure.
Possibly the excavation crew did a--/u/me the line was old and properly abandoned in place.
In my opinion they should have done due dilligence to determine the source and status of the line.
Regardless, I sincerly believe that a proper lock out/tag out of the root valve when the well was first shut in should have been implemented.
LOTO would of/should of raised a red flag when the well was brought back on.
My own opinion.
According to official reports the source of fuel that caused the explosion came from a 1" line.
The line was plastic (poly?) and it had been cut 5' from the destroyed home's foundation.
The line was associated with the nearby APC owned well.

That's true. Sothern HSE director has lied to get rid of several people. Including his predecessor.
Remember, Anadarko cares about safety of its employees and the public, BS. They don't care, and now it's in the news. How much crap needs to blow up? Update your death insurance, and don't call out safety, or they will just retaliate and find a way to get rid you.

According to Denver Post and other reports the well did not blow up.
And there was a report of a neighbor stating the two men that were killed were installing a water heater.
The actual cause is still under investigation.
A CO well blew up and leveled a nearby home. Killed 2, injured 1. They claim they are going to analyze integrity of the other wells in the area, but I'm sure they'll be back in production before a proper analysis can be conducted. 2 weeks to conduct integrity analysis on 3000 wells... sounds like it will be really thorough.
