Nailed it. Been here since 2011. I've seen it first hand. I always preached and helped create specs for LOTO, but when there's no accountability or reprimand the result speak for itself. I know the haphazard games this place plays when it comes to so-called safety. Whoever wrote on an earlier post that this place is safer than Intel must be smoking something real good. I worked at Intel for 12 years, they were very strict when it came to safety. They had actual structure when it came to ergonomics, environment, safety, and health.

All of these companies abuse the Hot Work Rule. It is supposed to be reserved for use only when shutting the circuit down creates a hazard, not just out of convenience. They should ask themselves about the consequence (and cost) of failure vs. developing a method to de-energize the system. The results of that calculation should have been evident in this case, getting an employee injured and getting a hefty OSHA fine (plus the costs associated with the incident) far outweigh the cost of scheduling a shut-down.

Apparently, the layoff whiner bombarders here are not familiar with “hot work permits,” which are granted only when LOTO would cause major disruptions. Ask any electric company, for example. “Sorry, we have to power down all of Burlington to change a transformer.” Make sense?

This is not exactly breaking news. Years ago I was the victim of GlobalFoundries gross lack of safety and their attempted coverup. I was offered a large payout and amiable separation to not pursue the claim further in courts or regulatory agencies. I took it, recovered with lasting effects but life goes on much better than working in that he-l hole. Every worker should look around and desire something better. It’s out here. Join us.

Is the individual at blame for not following company protocols or were the protocols followed and still a safety issue happened? What could OSHA do to GF and how could it impact GF?

Article doesn't give much information

"Edmund Fitzgerald, a spokesman for OSHA, said an investigation has been opened into the incident, but he could provide no other details.

"That’s all the information we have at this time," Fitzgerald said.

OSHA has jurisdiction over worker conditions and safety programs at more than 7 million employers across the U.S. Companies must report all work-related employee inpatient hospitalizations to OSHA within 24 hours. OSHA can fine companies over certain incidents.

Officials with the town of Malta, as well the Malta Ridge Volunteer Fire Co., which originally responded to the incident before being called off, both said they had no details on the incident.

GlobalFoundries makes computer chips at Fab 8 using a process that involves vast amounts of electricity, natural gas and chemicals that are used to make the chips on 12-inch silicon wafers.

Global Foundries does not practice the safety that they preach.

They have unqualified employees running the entire safety group.

Documentation is very lacking which doesn't matter because no one bothers to read them, and no one cares.

Employeess don't show up for LOTO training yet perform tasks anyway.

Maintenance people continue not to use p-e task planning, yet when something goes wrong there's no reprimand.

The safety score would be worse if all the accidents were actually reported.

LOTO procedures not followed. Equipment that was supposedly deenergized was not.

What exactly happened to this employee? I'm not in Fab 8 location and I still work from home, so I haven't heard any details on what happened here? Was it an individual mistake or was it a company mistake by not having right business process in place?
