Has anyone been ranked as Above and Beyond on their review? I’m genuinely looking to find out what one would have to do to be ranked as such. I asked my manager but she is always vague in her answer and just gives generalized answers.

You could cure cancer, start a Mars colony, and find the fountain of youth and it wont matter. Its all on your manager and how they present you up the chain. I left because of poor management.
I have, two times, but I have received pay increase and promotions even when I was "only* fully met. Also, I had more than 100% most of the times even with fully met only... And you can have less than 100% with fully met marks too... It's a random circus
It depends on manager but also your peers as like it or not, you are compared to them. Additionally A&B is limited by % so depending on team size, the manager may be minted in how many receive A&B in their team.
