From what I gather, these systems often have too many chiefs & not enough Indians. Those who care for the sick, elderly, children and the mentally ill should always be regarded in higher esteem than the paper pushers who are often the ones causing these caregivers grief.

Expensive for payers of sick, elderly, children and the mentally ill who go to hospitals without citizenship. Usually goes unpaid unless taxpayers keep súcking up the costs.
