To me it’s funny how it’s the same questions year after year. The questions are oriented to not give quantitative results. Notice there isn’t a question that says “do you feel like you are compensated enough”…..
As a low level manager, I can tell you it is anonymous.
Your manager only sees the aggregate responses of their team if they have 7 or more people under them. They see how many “Strongly Agree”, “Agree”, etc. responses each question got, but can’t get any more detailed than that.
The only way a manager would know what you answered is if you’re already very outspoken about how you feel, and you are an outlier. If it’s a small team, and you’re the only one with a negative (or positive) attitude, and everyone else answers the opposite, it could be easy to guess… but that would already be the case.
Or they could just blindly guess, and see your reaction. Pretty lame in my opinion.
I guess it’s theoretically possible people way above my pay grade secretly get results with names, but at that level, if you think they care what any individual person answered enough to take action against them (which would mean giving up the secret all the way down to your direct manager)… you got other problems.
What about the bull action items they come up with after the fact? Waste of time
It isn't anonymous. I personally do not care to hurt some feelings , I said what I said about the previous pulse. But my manager told me about my output and how to improve. How did he know it was me?

Yea, right anonymous. I got something to say to Upper Management. Get off your high horse and come out of your ivory tower and quit giving me the BS comment "We look at things from a high level" when I ask for help. When you look at things from a high level, that little spot on the ground is a big pile of cr-p that is about ready to avalanche the workforce. So keep preaching "Everything is wonderful" and continue the lie from your ivory tower.

The survey isn't built for you to give real feedback. But since you're concerned I think most people answer in the negative where they feel it is justified.
It exists for HR to justify its existence and for HAL to pretend it's a GPTW. wE LiStEn.
