Also, check to see if you have Walmart Critical Illness Insurance through Allstate. If your spouse is covered, you can possibly file a claim. Allstate has actual customer service and functional website when compared to Sedgwick. 1-800-514-9525
If you call Sedgwick and go through the menu to get to a person they are super helpful explaining about the forms. There is a part the associate fills out and part the medical provider fills out.
Make a phone call to Sedgwick and ask him to ask his doctor's office to fax the necessary forms to the office so that they can be filled out.
They can be brought to the office in the quickest possible manner by using this method.
Make sure that you have the phone number as well as the fax number for the doctor's office.
You would be the one filling out the claim, it has to be done.
His doctor would need to do all the paperwork.
