I participated in this from the IT side. Overall the warmdown went well. Most of the planned actions were successful, which was a miracle (actually a result of several engineer's hard work). And where we saw problems, those issues could have been prevented with better planning and management. I was going to list out everything but there's no point.

Ops director learnt it from the now CEO past GM who tries to intimidate someone at every opportunity. Just have a look at interviews that he has been giving on TV tries so hard to keep himself in check

They screwed up gutting IT before a scheduled IT upgrade. Go call Infosys for help.
There is definitely a lot of middle management lying to higher levels or dressing up problems to be nothing. I was involved with a big issue last year where middle management was straight up lying and Misrepresenting what was going on to higher levels. Which just resulted in more headaches and churn
The reason is because in the planning even though this was mentioned to get better delivery the upper management decided we will recover the delivery after recovery so now desperate to get back so that they can hit their Q2 delivery numbers
