I’ve heard mixed feelings about self ID for disability, now with the email that HR just sent out… is this something that is ok to fill out? Or does this put you on a potential firing squad target list?

If possible, keep your name off of lists.
My wife at another company was forced out when she started having medical issues.
Don’t tell them ahead of time. When you need it make sure you have everything documented and go strong with the issue don’t tip toe around like my wife did because that’s when they push you out.
Also make sure your maxed out on long term disability insurance. This way if you can’t work you can / will get it.
I've worked 23 years with the company and they don't even know my real name.
Unless there is an immediate need for disability accomodation, let it ride.
What you want to do is work for a company where you don't need to worry about it

That is EXACTLY what happened to me as well. When I needed “assistance” - Manager suggested I find a solution.
Disability might flag you ask a risk or high cost so ranking may drop.
That really shouldn’t be the case. I’m know it’s hard but assume good intentions. Folks with disabilities have been widely excluded from I&D and they are trying to correct. Well and flaunt our metrics to the rest of the world.
Only fill it out if you really need to. I started to type out what happened to me involving disability, money and lawyers. I decided to erase it for obvious reasons. Tread carefully. I will say my ranking fell off a cliff and stayed there until I got PIPed out first year I was RE. It's no joke.
