TERMINATED EMPLOYEES: To access your W-2 from home, use www.mytaxforms.com and logon using Company Code 11116, a 10 digit number comprised of the number "9" followed by your SSN, and your PIN. If you do not remember your PIN, click on the "forgot PIN" feature. If you are a first time user, your PIN will be the default PIN equal to an 8-digit number made of the last 4 digits of your SSN plus the 2-digit month (MM) and 2-digit year (YY) of your birthdate. For example, if the last 4 digits of your SSN is 6789 and your birth date is 01-23-1975, your default PIN is 67890175. You will then be prompted to enter personal information that will be used to help confirm your identity. Once done, the system will send you a one time code via the method of your choosing. Once you receive the One Time Code, enter it and proceed to your account.
